“A COMPARATIVE CLINICAL STUDY OF SUBARACHNOID BLOCK WITH 0.5% HYPERBARIC BUPIVACAINE 10MG AND 0.5% ISOBARIC LEVOBUPIVACAINE 10MG IN PATIENTS POSTED FOR ELECTIVE CAESAREAN SECTION. A PROSPECTIVE RANDOMISED STUDY”

Abstract
AIM&OBJECTIVES:
To compare and evaluate the efficacy and safety of intrathecal Isobaric Levobupivacaine 0.5% 10 mg with Hyperbaric Bupivacaine 0.5% 10mg for cesarean section. To compare the sensory and motor blockade characteristics, recovery profile, duration of analgesia, and side effects between the two groups.
MATERIALS & METHODS:
100 Pregnant women of ASA I and II posted for elective lower segment cesarean section. The parturients were randomly allocated into two groups.
Group L (n = 50); 10 mg 0.5% (2 ml) levobupivacaine
Group B (n = 50); 10 mg 0.5% (2 ml) bupivacaine
All the sensory and motor parameters were assessed.hemodynamic stability and adverse reactions were compared between the two groups.
RESULTS:
the comparison of Time of onset of sensory block was significantly faster in group B (1.9
± 0.41min) than group L (2.16 ± 0.50 min). the Time to reach maximum sensory block was significantly shorter in group L (11.38±1.87min) as compared to the group B (13.42±1.01min).Time to two segments regression was significantly shorter in group L (69.00± 3.15min) compared to group B (89.40 8.08 min).the Time of onset of motor block was significantly faster in group B (2.88 ± 0.52 min) compared to group L (4.62 ± 0.90 min). Time to reach maximum motor block was earlier in group B (6.48 ± 1.20 min) compared to group L(10.98 ± 2.32 min).the comparison of duration of motor block was shorter in group L (100.96 ±1.30 min) compared to group B (134.9 ± 4.15 min).
The adverse effects were lower in the levobupivacaine group than bupivacaine group
CONCLUSION:
We conclude that 0.5% Isobaric levobupivacaine (10mg) could be used as a good alternative to 0.5% hyperbaric bupivacaine (10mg) in spinal anesthesia for a cesarean section as Levobupivacaine provided the shorter duration of the motor block that will allow early motor recovery, early ambulation of parturient and less hospital stay with better hemodynamic stability and lower adverse effects.
