A Comparative Study of Interscalene Block Using Dexmedetomidine With Bupivacaine And Clonidine With Bupivacaine.

Abstract
OBJECTIVE:
To compare the effects of addition of Clonidine and Dexmedetomidine as an adjunct to Bupivacaine for Interscalene block with respect to onset and duration of sensory and motor block and duration of analgesia.
MATERIALS AND METHODS:
70 patients of ASA 1 &2 scheduled for various orthopaedic surgeries for upper limb under interscalene brachial plexus block were divided into two equal groups(n=35)in a randomised double blinded controlled study .
Group I received 0.5% Bupivacaine (20ml) +Dexmedetomidine(1mcg/kg).
Group II received 0.5% Dupivacaine (20ml) + Clonidine(1mcg/kg).
RESULTS:
The mean onset time of sensory & motor blockade was faster in Dexmedetomidine group. (3.54, 5.40 respectively). The duration of sensory & motor blockade was also prolonged in this group (p value 0.006; &<0.001).Heart rate & mean arterial pressure in two groups were statistically not significant. Bradycardia &Hypotension seen in Dexmedetomidine group were clinically insignificant.
CONCLUSION:
Dexmedetomidine with Bupivacaine has good efficacy than clonidine with bupivacaine in interscalene block. Dexmedetomidine is extremely effective in reducing the time of onset of sensory and motor blockade and in prolonging the duration of block with less incidence of bradycardia and hypotension in the used dose.
